Thank you for transfer your patient Musat Paul-Mihai, born on 12.02.2002, of whom we inform you in the following.
Indication: Stereotaxic planification, glioma of the brain stem before removing, Biopsy.
NMR-NC from 13.08.2010
Technique:
NMR 1,5. 2 mm ventral T2w TSE, 2 mm ventral T1 endemic MPR and KM, reconstruction on several levels, 8 ml Magnevist i.v.
Result:
Inhomogeneous mass partially imposing cystic with solid isolated segments accentuated through substance of contrast in cerebral bridge, accentuated on right side by circa 4,3 x 3,1 x 3,3 cm, the mass reaching magnum foramen, involvement of vestibular nerve. Compression of the forth ventricle with a low enlargement of the lateral ventricle. Displacement of Artelia basilaris towards left, displacement of right Artelia vertebralis downwards and possible its compression.
Summary evaluation:
Solid cystic mass, with partial absorption of the substance of contrast, accentuated on left cerebral bridge, involvement of Nervus vestibularis, displacement of Artelia basilaris towards left, displacement of right Artelia vertebralis downwards and possible compression, the delimitation of bone structure is not possible.

Indication:
Pre-operator control for tumor of brain stem.
Endemic CCT from 22.08.2010
Technique:
Scanner Siemens Somatom 4+ Volume-Zoom. Limited evaluation because of a defection of the machine.
Result and summary evaluation:
Brain stem distention and compression of the forth ventricle of the brain stem tumor (see exam NMR from 13.08.2010). No indication about bones modification on block level or on the skull base level.

Clinical information:
Tumor of brain stem without substance of contrast.
Results:
Four endemic biopsies with sizes up to 1 mm.
Three segments of tissue requested for histological exam, one segment of tissue primary fixed in formalin and encrusted in paraffin.
Histopathological Diagnose:
Neuropitelial tumor of reduced level (Dr. Lüdemann, hour 12:34).
Histological (1 frozen block, 1 paraffin block p=g, 1 block of paraffin after primary apposition in formalin, respectively Masson-Goldner). Central nervous tissue from grey and white matter. In grey matter there are recognized numerous nervous cells predominantly large with flat cytoplasm through inclusions of Nissl substance, as well as vesicular nucleus and protuberant nucleolus. Moreover, white substance which is imposing slightly central with a high number of cells, without tumor cells clearly drawn.
Regarding immune histochemistry (respectively GFAP, MAP2, sinaptofizina, PS3, K157), a strong MAP2 reactivity in the region of grey matter, with marking of neurons and neuronal projection to negativity in the region of white matter. To GFAP reaction indicates isolated reactive astrocytes with cytoplasm slightly flat and protuberant projections. In the region of white matter a slightly diffuse reactivity appears. The tint of sinaprofizina corresponds to the MAP2 tint. No indication regarding cells with a P53 nucleus accumulation. Tint K167 indicates in both concoctions, repeatedly, marked isolated nucleus.

Tissue from grey and white matter with discrete gliosis and indications of some shaped isolated cells. (see comment).
Comment:
Indication regarding shaped isolated cells raises the suspicion that here appears the edge of a tumor. We will present the case to the reference center for cerebral tumors from Bonn , and after the entrance of diagnose we will take the necessary measures.
